Well, my staff – the fact that they can go anywhere in that system and it's just there isn't any place that you can hide.  You can't hide what you are doing; you can't hide what you miss.  You can't hide – if I need to, if I am checking out a patient and I forgot to write a prescription, I can still write a prescription for them.  I can still send an electronic prescription from the checkout screen.

If the insurance changes midway through the note, I can change that mid screen.  If I am working on a patient's note and I realize that I am documenting something that really doesn't fit in the template, I can do on-the-fly template editing.  I don't have to exit out of the patient's chart, open up another screen to open up a template and then go back into the patient screen.  I can do it right then and there in that one screen.

A lot of the base templates I have had to adapt to the way that I document just because it's a template, it doesn't mean that this is how it's supposed to go.  Every state, especially as podiatrists, we bill a lot to Medicare.  So there are different Medicare contractors around the country, each with their own local coverage determinations, each with their own little subset rules that you can only do X procedure on this date and this on this date.  So there are a lot of parameters that have to be set within the system relative to where you are practicing.

And also as you develop new technologies, you'll be able to document those new technologies whether you are doing ultrasound therapy, whether you are doing ultrasound imaging because those types of things didn't exist.  I had to create all of those on my own.  Adding more x-ray templates based on diagnoses, being more descriptive for more of the soft tissue procedures that need to be put in for incisional biopsies and soft tumor mass removals, being very descriptive.  In addition to adding home health, things for home health maintenance and management in addition to also making the changes with homecare.  As the codes change, as the descriptors change, we have to change description to go with the new descriptors.

I found it very easy to do that.  I did have to go through the training.  You don't do anything without training on these systems because they are just monstrosities.  But going through the training did help kind of stabilized down how I needed to lay out the templates.  It's just – the system is just so versatile that being able to navigate to multiple areas just makes it, the whole design of the system is to be completely effective for a single user.
